The Company Verification ID can be found by Company Card Administrators in MiVision under Company Info, or in your Welcome Email.

If your company pays for your card expenses, your credit limit will reset on the billing date. If you pay for your card yourself, the credit limit will reset on the Payment Due Date.

CSV stands for Card Security Value. CVV stands for Card Verification Value - an extra security feature of all credit and debit cards. It's the three-digit number printed on the back of your card, at the top right of the signature field.
